 If you ever have a female patient with a severe pes planus who needs the support of a medial flange but who also wants to wear dress shoes, consider ordering a polypropylene orthosis with a very shallow heel cup and medial flange. It is, in fact, not usually the arch that limits orthotic fit in the shoe but rather heel cup depth. Thus, a good option for these patients is to order a device as shown below. You can see this is a direct mold polypropylene orthoses with a 4 mm heel cup but with a medial flange. We are also able to grind the heel contact point almost paper thin to allow the device to sit very low in the shoe.
