Casting - The Critical Component for Custom Orthoses

To make the most effective custom orthoses, it is essential to start with quality casts. Depending on the type of orthoses, ProLab recommends the following casting techniques.

Functional Foot Orthoses and Functional AFO - ProLab recommends a nonweightbearing, neutral suspension casting technique with either the hallux dorsiflexed or the first ray lightly pressed to eliminate supinatus. AFO casts must capture the malleoli.

Accommodative Foot Orthoses - ProLab recommends a semiweightbearing casting technique. Foam impressions are acceptable for accommodative foot orthoses.

Stabilizer Gauntlet AFO - ProLab recommends a semiweightbearing casting technique on 1/2" heel-raised footboard, unless the diagnosis is instability of the STJ. For those patients, we recommend a subtalar neutral casting position. Casts must be at least 1" higher than the AFO height.
